Renew your IQP registration
Use this form to renew your IQP registration every three years. You can also request to remove or amend a specified system with this form but you cannot add a new one.
There is a fixed fee to renew your IQP registration.
You should know
Before you start
As you cannot save this form, you'll need to make sure you have the following documents handy:
- your insurance certificate, which must be current and not expired
- copies of any professional memberships or licenses
- your professional development log
- copies of any refresher courses or training you have undertaken in the last three years
- a recent photo of yourself for a new IQP membership card
