Before you start

Use this form if you have lost your dog tag and need a new one.

If you have moved house or would like to change your postal address, you will need to update your details before requesting a new disc or strap. You need to do this to ensure your dog tag is sent to the right address.

You need to be the dog owner to complete this form.

Make sure your dog registration is up to date before ordering a replacement tag.

The cost for each individual tag is $12.00

Only one replacement tag per dog is permitted

Payment

You can pay by credit or debit card or Account2Account. A card payment fee of 1.75 per cent will apply for credit or debit card payments.

Your privacy

We will use your personal information to authenticate you and to allow you to complete changes online. We will share your information with a third party only if we are required to by any laws or regulations. This includes meeting our obligations under the Dog Control Act 1996.
